Abstract: | This quasi-experimental research aimed to compare knowledge, perceived severity, perceived susceptibility, and outcome expectancy on hypertension of patients with hypertension before and after participation in a health behavior modification program; and to compare self-care behaviors and blood pressure levels of the patients before and after participating in the program. The population was 171 hypertension patients in Na-in Sub-district Promoting Hospital's responsible area aged 35-39 years. The sample comprised of 30 hypertension patients aged 35–39 years in the area obtained by simple random sampling. The instruments used were a questionnaire, health behavior modification program, and a blood pressure record form. The statistics used for data analysis were frequency, percentage, mean, standard deviation, and t-test.
The results revealed that the control group had significantly higher knowledge of hypertension control after participating in the program (t =-9.777, p.001); significantly higher perceived severity, perceived susceptibility, and outcome expectancy on hypertension (t =-8.620, p.001); and significantly higher self-care behaviors (t =-10.061, p.001), which resulted in better systolic and diastolic blood pressure. การวิจัยครั้งนี้เป็นการวิจัยแบบกึ่งทดลอง มีวัตถุประสงค์ 1) เพื่อเปรียบเทียบความรู้ การรับรู้ความรุนแรงของโรค การรับรู้ต่อโอกาสเสี่ยงของการเป็นโรค และความคาดหวังในผลลัพธ์ เกี่ยวกับโรคความดันโลหิตสูงของผู้ป่วยโรคความดันโลหิตสูง ก่อนและหลังการได้รับโปรแกรม 2) เพื่อเปรียบเทียบพฤติกรรมในการดูแลสุขภาพตนเอง และระดับค่าความดันโลหิตของผู้ป่วยโรคความดันโลหิตสูง ก่อนและหลังการได้รับโปรแกรม โดยทำการศึกษาระหว่างเดือนมีนาคม - พฤษภาคม 2564 กลุ่มตัวอย่างที่ใช้ในการวิจัย คือ ผู้ป่วยโรคความดันโลหิตสูงที่มีอายุ 35 – 59 ปี ของตำบลนาอิน อำเภอพิชัย จังหวัดอุตรดิตถ์ จำนวน 30 คน ได้มาโดยใช้วิธีการสุ่มอย่างง่าย เครื่องมือที่ใช้ในการวิจัย ประกอบด้วยแบบสอบถามที่ประยุกต์จากทฤษฎีแรงจูงใจเพื่อการป้องกันโรคร่วมกับทฤษฎีการดูแลตนเอง เพื่อนำมาออกแบบ โปรแกรมการปรับเปลี่ยนพฤติกรรมสุขภาพ แบบสอบถาม และแบบบันทึกค่าระดับความดันโลหิต ซึ่งได้ทดสอบค่าดัชนีความสอดคล้องของจุดประสงค์และเนื้อหา มีค่าระหว่าง 0.67-1.00 และได้ทดสอบความเชื่อมั่น มีค่าระหว่าง 0.70-0.86 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา เช่น ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงแบนมาตรฐาน และการเปรียบเทียบความแตกต่างค่าคะแนนเฉลี่ยก่อนและหลังโดยใช้สถิติ Paired t-test ผลการวิจัย พบว่า 1) ภายหลังการได้รับโปรแกรม กลุ่มทดลองมีความรู้เกี่ยวกับการควบคุมระดับความดันโลหิตสูงเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (t=-9.777, p < .001) และการรับรู้ความรุนแรงของโรค การรับรู้ต่อโอกาสเสี่ยงของการเป็นโรค และความคาดหวังในประสิทธิผล เกี่ยวกับโรคความดันโลหิตสูง เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (t=-8.620, p < .001) 2) ภายหลังการได้รับโปรแกรม กลุ่มทดลองมีพฤติกรรมในการดูแลสุขภาพตนเอง เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (t=-10.061, p < .001) ซึ่งส่งผลให้กลุ่มทดลองมีระดับค่าความดันโลหิตขณะหัวใจบีบตัวและขณะหัวใจคลายตัว ลดลงอยู่ในระดับที่ดีขึ้น |
